CONSENT STATEMENT UNDER THE PERSONAL DATA PROTECTION LAW

Altium International Lab. Cih. A.Ş. (“Altium TR”), in accordance with the relevant provisions of the Personal Data Protection Law No. 6698 (“PDPL”) and within the framework of the General Disclosure Text on Personal Data Protection (https://jasem.com.tr/en/gdpr/disclosure-text) and the Altium TR Personal Data Protection and Processing Policy, hereby informs you as follows:

Your personal data may be processed by Altium TR as the data controller or by authorized data processors appointed by Altium TR who take the necessary security measures;
This may include but is not limited to, your identity information, address details, contact information, and other personal data directly or indirectly related to the following purposes:

• To provide the products and services offered by Altium TR to its customers under the best possible conditions,
• To ensure the reliable and uninterrupted supply of products or services,
• To maximize customer satisfaction,
• To facilitate payments,
• To perform various transactions related to the aforementioned services, to conduct and improve business operations,
• To carry out promotion, marketing, advertising, and campaign activities of the mentioned products and services or different products and services,
• To inform customers about opportunities, campaigns, and other services,
• To fulfill the requirements of contracts concluded with customers.

Personal data may be processed in compliance with the general principles stated in Article 4 of the PDPL, primarily the principle of retention only for the period necessary for the purposes they are processed or as stipulated by legislation;
Personal data may be obtained, recorded, securely stored in physical or electronic media for an appropriate period according to the purpose of processing, retained, modified, rearranged, disclosed, transferred, acquired, classified, processed, or its use may be restricted in accordance with the applicable legislation.

I hereby acknowledge that I have been informed by the Company regarding the above matters and that I give my explicit consent within the scope of the Personal Data Protection Law.

I hereby acknowledge and declare that my personal data, provided that they are used strictly for the purposes stated above, may be transferred by the Company to Altium TR employees, officers, group companies (the Company and/or its business partners, shareholders), legally authorized public institutions and organizations, independent audit firms within the scope of legal obligations and restrictions for the purpose of conducting its activities, as well as to business partners and service providers from whom Altium TR receives services or collaborates for the execution of the services and/or activities to be provided to me, and that I give my explicit consent regarding this matter.

Furthermore, pursuant to Article 11 of the Personal Data Protection Law (“PDPL”) and relevant legislation; I acknowledge that I have the right to apply to the Company and:

• Learn whether my personal data is processed,
• Request information if my personal data has been processed,
• Learn the purpose of the processing and whether my personal data is used in accordance with this purpose,
• Know the third parties to whom my personal data is transferred domestically or abroad,
• Request correction of my personal data if it is processed incompletely or inaccurately,
• Request deletion or destruction of my personal data if the reasons requiring its processing no longer exist,
• Request that the correction and deletion requests be notified to the third parties to whom the personal data has been transferred,
• Object to any outcome arising against me by means of analysis of the processed data exclusively through automated systems,
• Request compensation for damages in case I suffer damage due to unlawful processing of my personal data.

I acknowledge that in order to exercise these rights, I have the right to submit my request containing necessary identity information and explanations regarding the right I wish to use to [[email protected]] via email, or in person to the Directorate of Altium International Lab. Cih. A.Ş. at Barbaros Mahallesi Temmuz Sokak No:6 34746 Ataşehir/ Istanbul with identity documents, or through a notary, or by other methods stipulated in the PDPL.

I also acknowledge and declare that the personal data I have shared with the Company is accurate and up to date; and that I will notify the Company of any changes to this information.

I hereby give my explicit consent to the processing, use, and sharing of my relevant personal data—including special categories of personal data as defined in the PDPL—limited to the purpose of processing within the relevant process, and to its retention for the necessary period. I confirm that I have been duly informed in this regard.

I further declare that I have read and understood this statement, the Company’s Personal Data Protection and Processing Regulation, and the Disclosure Statement.
